5th Grade and Under*

* For the 11-12 school year

This league is a 6-week program designed to introduce the game of lacrosse with the majority of the time being devoted to instruction with a healthy dose of game play. will be taught offensive and defensive techniques, therefore full equipment is required.  Beginner players are welcome!

Rental equipment is available for $15 for the entire 6 week session and includes a stick if needed.  Mouthpiece is not included.

The league starts on October 17, 2011

Practice Location is Hot Shotz Sports Arena, Mt. Pleasant.

Releases:

US Lacrosse Membership expiration date should be November 30, 2011 or after.

I understand that participating in a sport, including lacrosse, involves an inherent risk of injury and although remote, the chance of fatal injury does exist. I acknowledge and voluntarily assume all risk of bodily injury and to exempt and relieve Westmoreland Lacrosse Association (WLA) and its agents from any liability for personal injury, bodily injury, property damage or wrongful death that may arise out of or in any way be connected with activities related to participation in an event or scheduled practice.

I hereby give consent as the parent or guardian for the registrant to participate in activities through Westmoreland Lacrosse Association (WLA).

I consent for WLA staff to seek emergency medical care deemed necessary to the welfare of the registered participant. I understand it is my responsibility as the parent or guardian to provide payment for emergency care if incurred and to inform WLA in writing of any changes to emergency contact information in a timely manner.
